Red Light Therapy for Pain: What the Research Shows

If you're dealing with chronic pain, you've probably heard of red light therapy — also called photobiomodulation or low-level laser therapy. But does the science actually back it up?

Short answer: yes, with real but important caveats. Multiple peer-reviewed systematic reviews and meta-analyses have found that red and near-infrared light can reduce pain and improve function across several conditions. The evidence isn't perfect — many studies have small samples or inconsistent dosing — but the direction of the research is consistently positive.

Here's what the research says, condition by condition.

Knee Osteoarthritis

A 2024 meta-analysis in Physical Therapy (Oliveira et al.) reviewed ten studies with 542 participants. It found photobiomodulation significantly reduced pain at rest compared to placebo, with a moderate effect size. The authors concluded PBM "reduces pain intensity in patients with knee osteoarthritis and may improve disability," though they noted the evidence certainty was very low and recommended it as a complement to other therapies rather than a standalone treatment.

A separate 2024 meta-analysis in Archives of Physical Medicine and Rehabilitation (Li et al.) focused on older adults and found significant improvements in WOMAC pain and function scores, plus increased knee range of motion, when PBM was added to regular exercise.

Tendinopathy

A 2021 systematic review and meta-analysis in BMC Sports Science, Medicine and Rehabilitation examined randomized trials on low-level red and near-infrared light for tendinopathy. It found meaningful reductions in pain and improvements in function across the included studies.

Chronic Pain (Broad)

A 2026 translational systematic review in eBioMedicine (Shedding light on pain management) screened over 7,700 studies and found light therapy was associated with improvements in pain-related outcomes, though results were inconsistent when dim-light controls were used. The authors called it a "promising non-pharmacological intervention" while noting more rigorous research is needed.

A 2026 systematic review in Frontiers in Integrative Neuroscience looked specifically at randomized clinical trials for chronic pain — fibromyalgia, peripheral neuropathies, orofacial pain, and musculoskeletal pain. Most trials showed significant pain reduction, especially in fibromyalgia and neuropathy, with a favorable safety profile.

Musculoskeletal Disorders Generally

A 2017 systematic review and meta-analysis in the European Journal of Physical and Rehabilitation Medicine found low-level laser therapy reduced pain in patients with various musculoskeletal disorders.

A 2026 systematic review in Sport Sciences for Health (Timimi et al.) pooled 53 studies with roughly 2,800 patients. It reported an average 32% reduction in VAS pain scores across knee osteoarthritis, tendinopathies, low back pain, and postoperative pain, plus functional improvements of 18–25%. No serious adverse events were reported.

Neck and Shoulder Pain

A 2024 comprehensive review in PubMed examined photobiomodulation for shoulder and neck pain and disability, finding supportive evidence for pain reduction and functional improvement in these regions.

Athletic Injuries and Return to Play

A 2024 meta-analysis in the Journal of Strength and Conditioning Research evaluated photobiomodulation for injured athletes and found it reduced pain and supported faster return to play when used as part of a recovery protocol.

Wound-Related Pain

A 2024 systematic review and meta-analysis in Cureus examined low-level laser therapy for skin wounds and found it improved both wound healing and pain management outcomes.

The Honest Caveats

The research is encouraging, but it's not a magic wand. Most studies note:

  • Evidence certainty is often rated low to moderate due to small samples and inconsistent dosing
  • Protocols vary widely in wavelength, intensity, and treatment duration
  • PBM works best as a complement to other therapies, not a replacement
  • Individual results vary based on the condition and severity
